A Bachelor's degree in a related field and a valid clinical license are also required. Advocate for members' needs and ensure they have access to necessary resources and services. Maintain accurate and up-to-date documentation of all member interactions and progress. Stay current on industry trends and best practices in behavioral health case management. Adhere to all regulatory requirements and company policies and procedures. Collaborate with other members of the healthcare team to coordinate care and ensure continuity of services. Attend regular team meetings and participate in training and professional development opportunities. Uphold the values and mission of the organization in all interactions and duties. Cultivate a positive and supportive relationship with members, promoting trust and rapport. Continuously evaluate and monitor the effectiveness of care plans and make adjustments as needed.
